Issue with Individual Customer Screen Subslots (address, telephone, email)

Hi,
Data entered in address / telephone subslots get wiped off, only in the following scenario:
As in OOB, individual customer detail view form has a slot to hold last or new request slot,
where summary information of the new request can be entered.
1. On Save Button Click (individual customer detail main toolbar → save) users receive a
form exception or user exception from serverside extensions.
2. User clicks on the caller binocular in request summary screen to create a new caller
and clicks save in modal popup to save the caller and chooses the same caller
to backfill the caller widget in the request summary.
3. User clicks save on the main individual customer detail main tool bar. Individual Customer
& request records saved properly. But all the information entered in the
address, telephone & email subslots of individual Customer detail view gets wiped off.
Subslots do not get wiped off, if there was no serverside exception in between.
Another strange thing is, during the save in the Modal dialog’s popup, the main screen’s
subslot informations also get stored in DB with no corresponding relation to the individual
customer (eventhough our state handling is only to Parent).
Has anyone seen this behaviour before? Any suggestions will help us and appreciated.
